How to Include Your Loved One in the Suffolk County COVID-19 Memorial
Include your loved one on the Suffolk County COVID-19 Memorial by filling out this form. You may also share a photo and your favorite story about the person.
After we receive your submission, county staff will write your loved one’s name on a ribbon and affix it to the memorial.
If you prefer to write your loved one’s name on a ribbon and affix the ribbon yourself: After filling out the online form and indicating in the section about ribbons that you wish to write on the ribbon and affix it yourself, you may visit one of two locations where ribbons and a structure to which you may tie it will be available:
outside or in the lobby of the William H. Rogers Legislature Building, located at the William J. Lindsay County Complex at 725 Veterans Memorial Highway (please use Smithtown as the city for GPS purposes)
the lobby of the Evans K. Griffing Building, located at 300 Center Drive in Riverside (use Riverhead as the city for GPS purposes)
Ribbons and markers will be available near the memorials Monday through Friday from 10AM to 4PM.
The two memorials will ultimately be combined into one and put on permanent display in Hauppauge.
